A PDF with photo and wiring directions is available for Series I/II guitar/bass 1/4" jacks.

Until the late 80's, most Series I/II instruments were supplied with a stereo 1/4" jack. After that, the default was for mono. The PDF above shows both styles of wiring.

If you can't use the PDF, here's the wiring spelled out by terminal. The terminal numbers are molded into the plastic housing of the jack. They are tiny, but you can see them!

mono 1/4" wiring:
1- black to battery 1
2- red ribbon cable
3- white/blue to pin 5 of XLR
4- 20K resistor to pin 2 of XLR and 20K reisitor to pin 3 of XLR
5- red from battery 2
6- orange ribbon cable
7- red/white to pin 4 of XLR
8- no connection

stereo 1/4" wiring:
1- black to battery 1
2- red ribbon cable
3- white/blue to pin 5 of XLR
4- orange signal to pin 2 of XLR
5- red from battery 2
6- orange ribbon cable
7- red/white to pin 4 of XLR
8- violet signal to pin 3 of XLR

grounding flag wiring same for stereo or mono:
yellow ribbon cable
black to pin 1 XLR
black to harness
red to battery 1
black to battery 2
(order is not critical)
