Side mounted output jack needed

I've got a set of Alembic Activators (Jazz sized) and the matching electronics (Vol/Pan/Filter) that I want to install into a bass with a side mounted jack (Hamer CruiseBass w/ 2Tek for those interested). A direct retrofit would be ideal, but I don't know if it exists. I'm at work now, but I'll upload the pics this weekend. So, can anyone point me in the right direction? The bass has cylindrical stereo phone jack on body side. Almost powered guitars and basses are switched as short circuit (ring to earth). But Alembic instruments use the switch jack. I think following conversion. It's all right? You can view and purchase one here.
