I bought this back in June locally. Looks like a koa top, zebrawood back and maybe walnut and maple neck.

It did not come with a wood-card. Here are some pictures:

Check out the crazy number of laminations on the back of the headstock. Were the gold tuners original?

Files from back then are pretty sparse, so there is no mention of the tuners. We typically used chrome for their durability, but I'm sure we used whatever we could find some days.

top: Koa
back: Zebrawood
body: Maple
neck: 5-piece Maple and Walnut
scale: 24.75" medium
fingerboard: Ebony
inlays: mother of pearl
originally made for LD Heater
birthday: not recorded

One previous registered owner in NJ, who bought it used in 2003.

Looks like you were correct with your wood guesses!

That would be the guitar sister to my 75-215 Series 1 Bass.
Same wood recipe.

Todd,
It looks like this guitar like my bass has the crazy swirls in the zebra wood.
This was considered a rare special piece of wood shared with a guitar made for Greg Lake. There have only been a handful of confirmed guitar / basses made with it.
